UCP Information
UCP of Santa Clara/San Mateo Counties, Inc. is a not-for-profit
corporation governed by a volunteer Board of Directors. Currently
the board is comprised of parents of children with disabilities, adults
with disabilities and professionals within the community interested in
promoting the missions of United Cerebral Palsy.

Passbook to Services Program
In collaboration with caregivers in both counties, the
Passbook to Services provides parents with a handy access guide to services.
There are information sheets that can be passed to medical professionals,
saving parents the time required each time their child visits their doctor
or other medical professional. The Passbook is also provided through
hospitals, the Regional Centers, and the Early Start Program. For more

Community Education Program
UCPA SC/SM Counties is dedicated to increasing awareness
regarding disabilities. Specifically, the outreach includes:

Presentations to service groups, elementary & high schools,
as well as other health care professional groups.

On site, in our office, we maintain a resource library on
disabilities, legislation, parenting, legal rights, community services,
community access, and advocacy. This library is open to the public.

Community Organization/Program Development
UCPA SC/SM Counties along with other public and private agencies,
are involved in an integrated child care model. Some of the areas
addressed are accessibility, housing, employment, transportation, Medi-Cal
and Supplemental Security Income(SSI). The goal of this inter-agency
collaboration is to increase and/or enhance the services provided to consumers,
making the services more user friendly and more accessible. Also
included are public and private agencies that address the needs of
persons with disabilities and their families.
